MyPractice Report: Primary Care
The MyPractice Primary Care Physician Report was developed jointly by Ontario Health and the ICES, in partnership with the Association of Family Health Teams of Ontario, Association of Ontario Health Centres, and the Ontario College of Family Physicians.

The MyPractice Primary Care Physician Report is available through MyPractice Primary Care Plus with Screening Activity Report data, a shared online platform that also houses the Screening Activity Report (SAR). The reports remain separate within the platform, while providing physicians with more streamlined access in one place. Register for ONE® ID to get access.

Family Health Team Executive Directors
Family health team reports are PDF file(s) and an Excel file that is emailed directly to executive directors biannually. This report is not available for anyone else to see other than the Family Health Team's executive director who requests it.
If a Family Health Team executive director wants to receive a report for our May release, the registration deadline is March 31, for the November release the deadline is September 30.
