The purpose of this dataset is to securely store quantitative data on Alternate Levels of Care (ALC) performance indicators specific to STTCMs and patient satisfaction.

This PHI is required to evaluate the STTCMs and identify the impact these models may have on local ALC pressures and patient outcomes. This information is used to design solutions to continue to reduce ALC volumes and support potential future implementation of these models.
