Home and Community Care
Ontario Health supports health service providers by providing resources and training to help optimize patient care. The Integrated Assessment Record (IAR) supports a client-centric approach to care by enabling access to assessment information that provides a holistic understanding of the client’s health care needs. Together, the Common Assessments and Integrated Assessment Record (IAR) enable collaborative care planning as well as enhanced communication between providers, for the ultimate goal of promoting high-quality care for clients in the community.
