eForms Ontario

eForms Ontario is a secure provincial digital solution delivered as part of the Patients Before Paperwork (Pb4P) initiative. With an initial focus on government administrative forms, eForms Ontario will give clinicians a simpler way to find, complete, and electronically submit forms – replacing paper-based and fax-dependent workflows.

The initial release of eForms Ontario provides the following benefits to clinicians, receiving organizations, and the health system:

  • Improves data quality and supports patient safety by enabling electronic submission of complete forms with built-in validation and standardized data fields, reducing errors, delays, lost information, and resubmissions associated with paper and fax-based processes.
  • Accelerates form submission and processing by replacing paper- and fax-based workflows with electronic submission, reducing delays associated with printing, scanning, faxing, and manual handling.
  • Reduces administrative burden by digitizing form completion and submission, eliminating printing, scanning, faxing, and follow‑up rework for clinicians and staff.
  • Enhances visibility and accountability by enabling submission tracking, auditability, and clearer oversight of form status and workflows across the system.
  • Improves service coordination by enabling more complete and standardized information to be shared between clinicians and receiving organizations, reducing follow‑ups and duplicate requests.
  • Strengthens privacy and security by replacing fax‑based processes with a secure provincial digital solution that supports identity, consent, and reliable information handling.

Clinicians can request access to eForms Ontario by contacting an OntarioMD Advisor, regardless of how they plan to use the service.

eForms Ontario can be accessed in one of two ways once onboarded:

  1. Access eForms Ontario through Electronic Medical Record (EMR)
  2. Access eForms Ontario through ONE Health Platform

Clinicians using the following EMRs can access eForms Ontario directly within their EMR:

  • Alpha Global – GlobeMed
  • Canadian Health Systems (CHS) – EMR Advantage
  • QHR Technologies – Accuro EMR

Clinicians using other EMRs or those without an EMR, can access eForms Ontario through the ONE Health Platform once it becomes available.

How to get started

To request access, submit an Expression of Interest by contacting your OntarioMD Advisor.

An OntarioMD representative will follow up with next steps and onboarding support, including confirming whether access will be enabled through your EMR and/or via ONE Health Platform.

Who is eForms Ontario For

Senders

Primary care providers who complete and submit forms as part of patient care and administrative workflows, including:

  • Family Physicians
  • Nurse Practitioners
  • Registered Nurses
  • Specialists
  • Other authorized care providers and designated clinic staff acting on their behalf, where permitted

Receivers

Organizations and authorized teams that receive, review, and process submitted forms, request follow‑up information, and update form status within the tool. This includes organizations such as:

  • Ontario Health atHome
  • Ministry of Transportation (MTO)
  • Other participating provincial programs and authorized recipient organizations

Get Started With eForms Ontario

1. Access the Tool

Users can access eForms Ontario through participating electronic medical records (EMRs) or through ONE Health for those without access to an EMR.

2. Sign In Securely

To use eForms Ontario, users must sign in with a valid ONE ID account to ensure secure access and protection of patient information.

Important: Having a ONE ID account alone does not grant access to eForms Ontario or other services. Access is only provided once your organization completes the full onboarding and validation process, which begins by contacting an OntarioMD Advisor at support@ontariomd.com. This process helps ensure clinicians receive the appropriate onboarding support, access configuration, and guidance needed to successfully adopt and use the service.

3. Complete and Submit Forms

Authorized users can launch a digital form, review pre‑populated patient and provider information, complete required fields, and securely submit the form to the intended recipient.

4. Track and Manage Forms

Submitted forms can be tracked in eForms Ontario, including confirmation of receipt, status updates, and follow‑up communication where applicable.

How Does eForms Ontario Work

eForms Ontario is designed to support a more efficient and reliable way to complete and manage administrative forms.

Key features include:

  • EMR integration: Launch eForms Ontario directly from a patient’s chart in participating EMRs.
  • EMR‑agnostic access: Access eForms Ontario securely through ONE Health when EMR integration is not available.
  • Pre‑populated information: Patient and provider information can be automatically populated into digital forms, reducing repetitive data entry.
  • Submission tracking: Users can view submitted forms and track key updates, including receipt confirmation.
  • Status visibility: Changes to form status are visible within the tool to support transparency and follow‑up.
  • Bidirectional communication (starting with the MLTC Health Assessment Form): Secure messaging between senders and receivers supports clarification, follow‑up, and status updates. This capability will be rolled out to additional forms as eForms Ontario continues to expand.

Available Digital Forms

eForms Ontario supports the following government administrative forms:

  • Ministry of Long‑Term Care (MLTC):
    • Health Assessment Form
  • Ministry of Transportation (MTO):
    • Mandatory Report
    • Driver Medical Report
    • Medical Condition Specific Report

The MTO Mandatory Report includes assessments for both Commercial and Non-Commercial Driver Medical Report.

The MTO Medical Condition Specific Report supports a wide range of medical assessments including:

  • Cardiovascular Assessment
  • Cerebrovascular Assessment
  • Cognitive Assessment
  • Seizure LOC (Loss of Consciousness) Assessment
  • Mental Health Assessment
  • Other Condition Assessment
  • Diabetes Assessment
  • Substance Use Assessment

More forms will be added over time as eForms Ontario continues to evolve and expand.

Secure Access with ONE ID

To safeguard patient information and maintain secure access, all users must have a valid ONE ID account. ONE ID is Ontario Health’s digital identity credential, enabling secure sign-in to its digital services.

Future Releases

eForms Ontario will continue to evolve to include new government administrative forms and enhanced capabilities that support more efficient and connected digital workflows for clinicians.

Future enhancements will focus on improving usability and integration, including increased auto-completion of form data from EMRs, expanded access across additional EMR systems, and the ability to attach files directly from EMRs. The solution will also support enhanced communication between clinicians and form recipients, co-authoring by multiple clinicians, and automatic saving during form completion.

As the solution evolves, these improvements will help reduce manual work, improve data quality, and streamline care coordination.
