Remote Care Management and Surgical Transition Funding

The Ministry of Health is investing up to $8.55 million to extend and scale programs led by Ontario Health Teams (OHTs) and other eligible organizations that provide remote care management and surgical transition programs to COVID-19 patients and other patient groups (i.e., mental health, frail seniors, chronic disease, etc.) that would benefit from remote clinical care and monitoring in the community during and beyond the pandemic.

Ontario Health is inviting proposals for one-time funding for the following:

  • Extended funding to continue and/or adapt existing programs that have demonstrated implementation success up to March 31, 2023
  • New remote care monitoring initiatives and surgical transition programs targeting priority patient populations that meet the funding criteria below

Who can apply for funding

  • Organizations that are members of an approved OHT
  • Organizations that are members of In Development Teams
  • All other health care providers and organizations who are not yet part of an approved OHT may submit a proposal that aligns with the program requirements outlined in the 2022-23 Funding Criteria document
  • Indigenous community organizations, including those serving First Nations, Inuit, Metis and Urban Indigenous persons

Proposals that support care for Francophone communities are encouraged.

Funding objectives

The objective of this funding is to ensure COVID-19 patients and other vulnerable patient groups receive appropriate clinical care and monitoring in the community, reducing the need for in-person interactions, and thereby reducing the risk of infection among patients and health care workers.

Successful proposals will demonstrate the capacity to provide remote care for both COVID-19 specific and other vulnerable patient groups which can include:

  • Probable or confirmed COVID-19 patients who are advised to self-isolate and/or confirmed COVID-19 patients (positive test result) discharged from an acute care setting who require additional clinical monitoring
  • OHT and/or regional priority patient populations (vulnerable patients) who would benefit from remote care monitoring and surgical transition programs. Potential patient populations may include but are not limited to racialized, patients in COVID-19 hot spot communities, frail seniors, patients with chronic conditions who would benefit from a palliative approach to care and individuals who require mental health support

Project requirements

All successful proposals must:

  • Be led by an OHT, In Development Team, or one or more health care provider organizations with the capacity to provide cost-effective remote care monitoring services
  • Propose a sustainable operating model with significant in-kind co-investment that is appropriate for anticipated patient volumes. Clinical staffing requests must be matched with clinical in-kind contributions
  • Be designed and governed with input from priority patient populations as outlined above and mechanisms to measure and improve the patient experience
  • Be well-integrated across partner programs and services to accept referrals, support clinical care and address health inequities for priority patient populations across one or more OHT regions or a broad sub-regional catchment area
  • Leverage proven remote care management and/or surgical transition programs, processes, and solutions for care of COVID-19 specific and priority patient populations
  • Be well-integrated with existing COVID-19 programs and services, including public health processes
  • Be supported by an OHT funder or a lead health service provider organization who would agree to signing a funding agreement with Ontario Health that includes performance targets and reporting requirements
  • Be willing to participate in an evaluation
  • Be reviewed, submitted, and endorsed by the relevant Ontario Health region
  • Acknowledge that funding is only available for the 2022/23 fiscal year

Proposal guidelines

Submissions from approved OHTs, In Development teams, and multi-OHT submissions should demonstrate that their proposals align with the OHT model. Examples of how applicants can demonstrate alignment with the OHT model include the following:

  • The proposal was developed by your OHT using your Collective Decision-Making Agreement (CDMA)
  • The proposal aligns with OHT priorities and OHT-specific performance indicators
  • The proposal involves an active cross-sector collaboration between OHT members
  • Funds will be flowed to and managed by the OHT fundholder on behalf of all beneficiary OHT members
  • The proposal aligns with Ministry direction to In Development teams (if applicable)
