Description of Services

Ontario Health eForms is an online platform for authorized users to submit electronic forms (for example, Health Assessment Form). This platform provides access to a library of digital forms through integration with electronic medical records. It provides features for pre-filling patient and provider demographic information, attaching files, sending email notifications for submitted forms, and tracking the status of these submissions.

Ontario Health has physical, administrative and technical safeguards in place to protect personal information (PI) and personal health information (PHI) against loss, theft, unauthorized access, disclosure, copying, use or modification.
