Location: Ontario Health Region office location with occasional travel in the region and to the provincial program offices in Toronto, assuming a hybrid work environment

Anticipated time commitment: 1 day per week, equivalent to 0.2 FTE

Term: 2 years + optional 2-year extension

Position summary

Ontario Health is seeking expressions of interest from qualified family physicians or nurse practitioners practicing in primary care for the position of Regional Primary Care Clinical Lead. This is a unique opportunity to collaborate with regional interest holders and provide clinical leadership to the evolution of primary care in Ontario. Reporting directly to the Vice President, Clinical Programs for the corresponding Ontario Health Region, with indirect reporting and accountability to the Provincial Primary Care Clinical Lead, this role plays an instrumental role in shaping regional primary care delivery systems and implementing regional initiatives aligned to provincial priorities.

Accountabilities

  • Provide clinical leadership and advice to Ontario Health provincially and regionally to support high quality, standardized, regional and provincial primary care program development and implementation.
  • Engage with, and build consensus among, regional primary care clinicians and interest holders to drive program evolution and adoption in support of achieving Ontario Health provincial and regional priorities for primary care.
  • Support the development and implementation of regional communication processes with primary care and interest holders.
  • Ensure there is a strong regional primary care foundation in place, including a Regional Primary Care table or similar body that can support: primary care service and program development, implementation, and quality improvement at a regional level.
  • Identify opportunities for capacity building within the regional primary care community and support the implementation of programs.
  • Work with regional leadership and relevant interest holders to identify and achieve region-specific goals and objectives.
  • Work with partners to drive implementation, monitoring, and improvement of regional activities in support of system-level provincial plans and initiatives. These priorities may include:
    • Ensure coordination and integration of care in the region by working with local partners and service delivery organizations.
    • Support the evolution of Ontario Health Teams (OHTs) and primary care networks (or similar structures).
    • Support targeted regional activities to address issues of capacity, access, and flow.
    • Increase equitable access to community primary care services for vulnerable and marginalized populations and other equity-deserving groups.
  • Represent Ontario Health at key regional and provincial clinical leadership tables and lead regional interdisciplinary collaboration to support capacity building in primary care.
  • Work with Ontario Health’s Provincial Primary Care Program to identify and achieve provincial goals and objectives.

Qualifications

  • Practicing family physician in good standing with the College of Physicians and Surgeons of Ontario, and either the College of Family Physicians of Canada or the Royal College of Physicians and Surgeons of Canada or nurse practitioner currently practicing in primary care in good standing with the College of Nurses of Ontario
  • A minimum of five years of clinical experience in primary care
  • Experience in a leadership role focused on quality and system improvements
  • In-depth understanding of Ontario's health care system, with a particular focus on the challenges and issues facing primary care in Ontario
  • Expert knowledge of current best practices, healthcare literature, and evidence-based guidelines, as it pertains to primary care generally
  • Knowledge of health system performance measurement and management approaches, and how this can be used as a tool to promote change
  • Aptitude for system thinking and ability to analyse information for sound decision making
  • Excellent communication skills with mature leadership experience in establishing strong relationships and building consensus with partners
  • Demonstrated commitment to equitable and person-centred care, as well as public accountability
  • Demonstrated experience in implementation of large-scale change in the health sector
  • Demonstrated ability to work with diverse interest holders to reach consensus
