Model of Care: Children Receiving Care in all Settings

The Pediatric Model of Care aims to provide guidance on how to optimize future palliative care delivery for children and their families in all settings. The model of care for children receiving care in all settings will ensure:

  • Children and families have access to high quality palliative care in their preferred location, as close to home as possible, enabled through a Hub-and-Spoke design.
  • Pediatric palliative care is provided by an Interprofessional Pediatric Palliative Care Team to enable high-quality, coordinated care across care settings, available 24 hours a day, seven days a week.
  • Children and their families are at the centre of the Interprofessional Pediatric Palliative Care Team and are actively engaged in decisions about their care, with attention to their values, wishes and preferences.

The Pediatric Model of Care encourages adaptation to local needs and resources, with a focus on enhancing current partnerships and collaborations, and establishing new relationships as needed. 

Engagement with First Nations, Inuit, Métis and urban Indigenous (FNIMUI) partners is required to develop Indigenous-specific Model of Care recommendations that reflect the self-determined priorities of FNIMUI peoples. These recommendations will be reflected at a future date, following the co-development process.
