eForms

Our eForms platform minimizes reliance on paper and fax-based workflows, eliminate redundancies caused by incomplete or incorrect information and offer a more secure and efficient way to manage patient data.

A provincial eForms strategy has been developed to:

  • enhance health system coordination
  • improve patient safety
  • optimize clinical workflows
  • reduce administrative burden for health care providers

To support the goals of the provincial eForms strategy, we have created an Ontario Health eForms platform.

Platform Features

  • Direct integration with electronic medical records (EMRs) - Clinicians, administrative assistants or other authorized individuals can launch the system directly from a patient's chart in the EMR.
  • Pre-population of information - Digital forms are opened in a separate browser tab with pre-populated patient and provider information.
  • Tracking and management - Users can view Health Assessment Forms previously submitted via eForms under the same health information custodian's authority.
  • Managing submitted forms - Users receive a confirmation notification upon submission. Submitted forms appear in a dashboard, displaying title, recipient, submission date and time, and status.

Platform Benefits

  • Efficient form submission - Administrative forms are sent securely to the intended recipient via the eForms platform.
  • Automatic notification - Recipients (such as Ontario Health atHome coordinators) receive notifications on newly submitted forms.
  • Status updates - Changes to form status, by either clinicians or recipients, are visible through the platform for all users.
  • Reliable transmission - Eliminates the inefficiencies of fax-based communication.
  • Enhanced accessibility - Clinicians can search and auto-fill forms with patient and or provider information.
  • Secure access - User access to digital forms is protected by the provincial ONE ID account.

Digitized Health Assessment Forms

The Ministry of Long-Term Care’s Health Assessment Form (HAF) is for completing the assessment required under the Fixing Long-Term Care Act, 2021 when a person applies for a determination of eligibility for long-term care home admission. 

The first limited production release of eForms focuses on digitizing the HAF and integrating it into point-of-care systems used by clinicians and Ontario Health atHome coordinators. The HAF is the first of 12 forms identified for digitization by the Ministry of Health and the Ontario Medical Association, with additional forms to be added as the platform expands.
