Eligibility and Selection Criteria

The 2024 CQCO Quality and Innovation (QI) Awards welcome nominations from any institution, practice or organization that provides cancer prevention activities or cares for people being treated for or living with cancer in Ontario.

The Quality Award is presented to a program or initiative that best demonstrates these criteria:

  • Improved outcomes
  • Sustainability
  • Economic benefits
  • Meaningful impact on patients, families, resources, and processes

The QI Awards define, and measure quality based on Institute of Medicine’s 6 quality dimensions:

  • Effectiveness: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).
  • Efficient: Avoiding waste, including waste of equipment, supplies, ideas, and energy.
  • Equitable: Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.
  • Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.
  • Safe: Avoiding harm to patients from the care that is intended to help them.
  • Patient-Centered: Providing care that is respectful of and responsive to individual patient preferences, needs and values, and ensuring patient values guide all clinical decisions.

The nomination must show measurable evidence of how the quality initiative improved outcomes. Upon preliminary review of your nomination, we may recommend that your submission be considered for a different award category.

By allowing themselves to be nominated, the nominees acknowledge and agree that – if they are selected for the award – their names and initiative will appear on the Quality and Innovation Awards Winners page. We may also feature winners on Ontario Health social media platforms.
