Eligibility and Selection Criteria

The 2024 CQCO Quality and Innovation (QI) Awards welcome nominations from any institution, practice or organization that provides cancer prevention activities or cares for people being treated for or living with cancer in Ontario.

The Quality Award is presented to a program or initiative that best demonstrates these criteria:

Improved outcomes
Sustainability
Economic benefits
Meaningful impact on patients, families, resources, and processes

The QI Awards define, and measure quality based on Institute of Medicine’s 6 quality dimensions:

Effectiveness: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).
Efficient: Avoiding waste, including waste of equipment, supplies, ideas, and energy.
Equitable: Providing care that does not vary in quality because of personal characteristics such as gender, ethnicity, geographic location, and socioeconomic status.
Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.
Safe: Avoiding harm to patients from the care that is intended to help them.
Patient-Centered: Providing care that is respectful of and responsive to individual patient preferences, needs and values, and ensuring patient values guide all clinical decisions.

The nomination must show measurable evidence of how the quality initiative improved outcomes. Upon preliminary review of your nomination, we may recommend that your submission be considered for a different award category.

1. Provide a summary of the entire submission including highlights of the initiative and why it should be considered for the Quality Award. Please also include: Highlights from the Background and Outcomes sections. (max. 350 words)

2. Describe the problem that led to the implementation of the quality initiative and how the initiative addresses the problem. Please also include: Description of the initiative. Who is the target audience (patients, clients, system, etc.)? When and how was it implemented? How long was the process? How does the initiative address the problem (use data)? Identify the quality dimension(s) it impacts and how? (max. 500 words)

Outcomes

3. Describe the initiative’s results, how it impacts patients and families, healthcare providers and the system, and how this impact is measured. (max. 350 words)

4. Describe how the initiative improves cost-effectiveness and value for money or improves quality without additional costs. (max. 350 words)

5. Describe how the initiative is being promoted and applied throughout the organization. Include specific efforts undertaken to share with others. What tools and processes are in place to sustain it or to ensure dissemination and uptake in other regions? Is it scalable across the province? (max. 350 words)
