Eligibility and Selection Criteria The 2025 CQCO Quality and Innovation (QI) Awards welcome applications from any institution, practice or organization that provides cancer prevention activities or cares for people being treated for or living with cancer in Ontario. The Innovation Award is presented to a program or initiative that best demonstrates these criteria: Originality and boldness Improved outcomes Sustainability Economic benefits Meaningful impact on patients, families, resources, and processes The QI Awards define innovation as new and unique processes, products or organizations that are bold, experimental, and far-reaching. The goal of innovation should be to improve the process, organization, or product. The nomination must show a plan for evaluation or measurable evidence for how the innovative initiative improved outcomes. Upon preliminary review of your nomination, we may recommend that your submission be considered for a different award category. The Quality and Innovation Awards defines, and measures quality based on 6 quality dimensions. The QI Awards recognize and encourage initiatives that improve the patient experience along the cancer care continuum, from prevention to survivorship or end-of-life care.
